needed.The Impact You Will MakeNurse Practitioner (NP) plays a key
clinical role in delivering compassionate, evidence-based care to
individuals with substance use disorders. The NP is primarily
responsible for the initiation and maintenance of
medication-assisted treatment (MAT), particularly for opioid use
disorder, within a collaborative care team. This role reports to
the Center/Area or Regional Medical Director.
- Provide ongoing medication assisted treatment for patients with
opioid and/or alcohol use disorder.
- Administer injectable medications including but not limited to
Vivitrol, Sublocade, Brixadi and antibiotics.
- Assess and select appropriate treatment regimens based on
clinical guidelines and individual needs.
- Collaborate with supervising physicians and care teams on
complex cases and care planning.
- Maintain accurate, timely, and complete documentation of
patient encounters (within 48 hours).
- Participate in interdisciplinary team meetings, staff meetings,
clinical training, and quality improvement initiatives.
- Assist with onboarding and mentoring of new clinical staff, as
requested.
- Maintain all required state licenses, national certifications,
DEA registration, and continuing education.
- Provide coverage at regional sites as needed for vacation,
illness, or staffing shortages.
- Engage in organization-wide meetings and initiatives, including
quarterly company meetings and committee participation.
- Provide clinical care using telemedicine protocols as
appropriate.
- Demonstrate consistent attendance, punctuality, and
professionalism.
- Perform other duties as assigned to support the mission and
goals of CleanSlate Centers.Preferred Qualifications
- Graduate of an accredited Nurse Practitioner program with
appropriate certification.
- Active, unrestricted license to practice in the state of
emploment.
- DEA license.
- Demonstrated interpersonal, communication, and organizational
skills.
- Demonstrated critical thinking skills.
- Proficiency with electronic medical records (EMR) and computer
sytems.
- Professional demeanor with the ability to work collaboratively
and adapt in a fast-paced environment.
- Receptive to feedback with a continuous improvement
mindset.
- Occasional regional travel may be required.Discover the
